Referral Procedure
Ages Birth to Three Years
Ages birth to three years:
If the family doctor or parents have concerns regarding the child’s development, parents may:
1. Call the Education Service Unit#3, In Omaha, 597-4936. Tell the secretary, Patti, the concerns for the child. A Service Coordinator will contact the family and make an appointment to visit at the home. She will explain the evaluation procedure from that point, which includes gathering information from the doctor and any other necessary source.
2. The Service Coordinator visits the home with a representative from Blair Schools for intake information and a parent interview which assists the family with a developmental checklist regarding the child. Then, if further evaluation is needed, then a team member will be calling the family to make an appointment to do so.
3. A meeting will be held to conclude the evaluation process. This is called a Multidisciplinary Team meeting. If the child qualifies, an IFSP will be written with the family’s goals.
